Waiver/Release Form for CareAlot Acres

I acknowledge and agree that I am participating in the events, activities, classes and workshops offered at CareAlot Acres and/or visiting and/or a guest of CareAlot Acres.  I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding participation in the activities offered at CareAlot Acres.

 

In consideration of being permitted to participate at CareAlot Acres, I agree to assume full responsibility for any risk, injuries or dangers, known or unknown, which I might incur as a result of participating in the program.  In further consideration of being permitted to participate, I knowingly, voluntarily and expressly waive any claim against the owner(s) and CareAlot Acres for injury or damages to self or property that I may sustain as a result of participating in the program.  I, my heir or legal representative forever release, waive and covenant not to sue the owners, representatives or CareAlot Acres for any injury or death caused by their negligence or other acts.

 

I hereby consent to and authorize the use and reproduction by CareAlot Acres of any and all photographs, video recordings, or audio recordings taken for the use on CareAlot Acres’ website, in print and other media for the purposes of promotion, illustration, advertising, or publication, without compensation.  All recorded media, prints and created media from the content shall constitute the property of CareAlot Acres.

 

Damages and losses by participants, visitors, guests can be sought by Carealot Acres and its owners/representatives.  Please be responsible.  Thank You.
